Community Nursing

The Community Nursing service provides nursing care for patients at home or in a care home.

We have an important role in preventing you from having to go into hospital as well as helping you to get back home after a hospital stay. We do this by helping you to manage your condition so you can stay as independent as possible.

We have several teams that work in different areas across Hull and East Riding who help with:

• Catheter care
• Giving medication
• Nusing care at end of life
• Bowel care
• Taking blood samples
• Wound care
• Support and advice for patients and carers to help you self-manage your care

To access the service you must:

• Be 18 and over
• Live in the Hull & East Riding area
• Be unable to attend a community treatment room for your care
