How can a Community Link Worker support you?

Often referred to as Social Prescribing, your Healthcare Professional refers you to a Community Link Worker who
can signpost you to a range of free local community groups, activities and services that provide practical, social or
emotional support. At the first appointment, your Link Worker will work closely with you on a one-to-one basis.
They will listen to you, support you to make plans and help you choose what will work for you to improve your
wellbeing, keeping you in control.

By signposting you to services in your local community, you will benefit from:

• Finding out what’s happening locally and meet new people
• Support with your money worries
• Support to continue/take up new hobbies and interests
• Support to improve your general health and wellbeing
• Reduction in isolation and loneliness
• Learning new skills and/or get into work

How do I get involved?

We can offer telephone appointments, appointments within GP surgeries and also home visits in some patient cases.

The Be Well Health & Wellbeing service covers the Venn Primary Care Network (PCN) which consists of the following GP practices:

  • CHCP: The Quays & Riverside
  • CHPL: Bransholme, Southcoates & Field View
  • The Bridge Group: Elliott Chappell & Orchard Centre
  • Sutton Manor Surgery
