Heart Failure

The Community Heart Failure Specialist Nurse Service provides support, advice, medication and reviews for patients diagnosed with heart failure. 

We help improve your quality of life and help reduce the risk of you being readmitted to hospital.

We can help with:

  • Reviewing your prescribed medication
  • Co-ordinating your care
  • Education and advice for patients, carers, families and other health professionals

Appointments are available in various locations across Hull and East Riding. If you are housebound, you can be seen at home.

How to access the service

You should be referred into the service by your GP or health professional.

Exceptions: If you have previously received support from the service and your condition becomes unstable, you or a family member can refer you back into the service.

To access the service you should:

  • Be 18 years old or over.
  • Be registered with a Hull or East Riding GP.
  • Have evidence of heart failure that meets the service criteria.
  • Patients who do not meet the criteria, but who have a heart failure diagnosis, may be considered after discussion by the team.
