Highfield

Highfield is relaxed and friendly, homely and comfortable. It’s not a hospital.

What is ‘reablement’?
A reablement service is provided either in a unit such as Highfield or in your own home. It is for short-term rehabilitation, and you will have regular assessments to decide how long you need the service for. This might be
for a few days or until your reablement is complete. The assessments will be done by the people looking after
you and others involved in your care and you will have regular reviews of your progress to check you’re happy to
continue your rehabilitation at Highfield. If you need long-term support, we may refer you to the
long-term team. This service is means tested so you may have to contribute to the cost.


Visiting times
2.30pm to 4.00pm
6.00pm to 8.00pm


Only two visitors are permitted per patient at one time. Please speak to a manager if you have any exceptional
circumstances and we will look into it.


Visitors
We encourage friends/family members to visit while you are staying at Highfield. Please supervise children at all
times and stick to the designated visiting times.

Infection, prevention and control
You may be asked to leave your room while it’s being cleaned to maintain our high standards of cleanliness
and to reduce the risk of cross infection. You may also be asked to leave your room before you are discharged
so that we can clean and prepare the room for thenext person.

Chiropody
We can arrange private in-house appointments for you on your behalf, but you are responsible for the cost.

Laundry
We don’t have the facilities to do your washing at Highfield so please arrange for your family or friends to do
this for you. Highfield is not responsible for any loss of or damage to clothing.

Pets
Your pets can be brought to visit you at Highfield, but they must be up to date with all their vaccinations and proof of this must be shown to the admin team.

Hospital appointments
Please let staff know in advance if you have any outpatient appointments at the hospital. Transport can be arranged
for you by the admin team/residential care leader. Please note that staff are unable to accompany you on
appointments.

Activity Co-ordinator
Highfield has a designated staff member to ensure all patients are offered the choice to socialise or have quality
one-to-one time throughout their stay.

What happens during my stay?
The emphasis is on you becoming as independent as possible during your stay in Highfield, through taking
part in therapy sessions, and following both the therapy team’s advice and healthcare assistants’ support to
improve your independence. Before you arrive please ask someone to bring in comfortable everyday clothing and some well-fitting footwear. You will also need to bring toiletries for your
own personal use. We recommend that you only have a small amount of money with you and that you leave valuables at home as we don’t have facilities for safekeeping. Highfield does not take any responsibility for any loss or damage. Any mobility aids (such as walking sticks, wheelchairs and Zimmer frames) must be agreed with an Occupational Therapist or a Physiotherapist and they must be brought in and labelled. Any continence equipment or aids to help you with daily living must also be brought in during your stay.

Medication
While you are staying at Highfield you may be required to temporarily re-register with a GP in the local area. This is
due to GP restrictions and boundaries for visiting. This will be sorted out by the care leader on duty. After your stay,
you need to re-register with your own GP. We have a full team of professionals including doctors, pharmacists and technicians to assist you in your health and the management of your medication and recovery while staying at Highfield. Please tell the care leaders if you are taking any over-thecounter medication or other medications from home, as they may affect your prescribed medication. While you are receiving care and rehabilitation at Highfield, your capability and health may improve greatly, and you may need to move rooms so that we can make sure our facilities and equipment meet your needs and those of other people using the service. The care you originally came to Highfield for may be reviewed as your needs may change.

Accommodation and facilities
There are 36 intermediate/reablement care beds at Highfield.


Single/double rooms with a sink, furniture and a TV. Some rooms have en-suite facilities and specialist
equipment such as hoists, commodes and handrails.


A rehabilitation kitchen will help you regain your skills in the kitchen so that you can go home with the skills you
need to live independently.


There is an on-call nursing system in every room for assistance and emergency calls only. Staff answer
the calls as soon as possible. There can be up to 36 patients at Highfield so it can become busy at peak times.


Communal lounges with TVs.


Falls safety bedrooms fitted with falls sensors on the bed/chair.


Large dining area where you can socialise with other patients while enjoying your meal, with doors on to a
secure garden area.


A large physiotherapy room to use throughout your rehabilitation journey.

Toilet, bathing, and shower facilities are available throughout the home.

Choices of menus at mealtimes.

Please tell the staff if you have any dietary requirements or allergies or any likes or dislikes.

Parking
There is free but limited car parking at Highfield. Please use designated parking bays and keep the main doors
and turning bays clear for emergency vehicles. You are responsible for the safety of your own property while your
vehicle is in the car park.

Access to Highfield
Please come through the main entrance – there is a bell on the wall to the right-hand side. There may be some delay in answering the main doors so please be patient.

Smoking
City Health Care Partnership CIC has a no smoking policy on all its sites in accordance with government legislation.
Smoking is not permitted on Highfield premises or grounds, except for patients only. There is an allocated smoking shelter at the back of the building located in the garden area, but this is strictly for patients only.

Alcohol and drugs
Alcohol and drugs must not be brought into Highfield. If you are found to be in possession or under the influence of these, you may be asked to leave the premises.

Fire procedure
We have a weekly fire alarm test and a monthly fire drill which is carried out by the care leaders. Patients are informed when the alarm is about to be tested to avoid confusion. If you hear the fire alarm, you must take immediate action and follow the instructions of the fire wardens on duty and the emergency services.

Discharge planning
This is a term used to describe the preparations for you to leave Intermediate Care or the Reablement Service.
We start thinking about this as soon as possible so that you can leave safely when you are ready. As your health
and abilities improve you may need to move to one of our other rehabilitation facilities.

Please note that we will not discharge you without a full discussion between Highfield staff, your family/carers
and yourself, or without the help and support that you may require.

Highfield is not able to offer temporary accommodation while re-housing issues are resolved.

Taxi service
A taxi can be arranged if you wish to attend appointments privately, but there is a cost for this service.


Safeguarding
Please speak with a staff member if you have any concerns/complaints about safeguarding and we will
contact the appropriate team for support/advice. If you need to contact the team directly you can
email chcp.safeguarding@nhs.net or call 01482 617875. Your mental capacity will be assessed throughout
your stay and recorded in your notes to ensure we are supporting all your health needs. An advocate can be
arranged on your behalf if needed. A Deprivation of Liberty may be required if you lack capacity, but this will be discussed with the people involved.

Business continuity plan
We have our own business continuity plan in place in case of any emergencies.
Zero tolerance
If you are verbally or physically abusive towards staff or others, services may be withdrawn from you, you may be
asked to leave the premises and the police will be called.

CQC

We are regulated by CQC and have regular inspections to ensure we are compliant with legislation and guidance
