Intermediate Care

Our intermediate care service provide a short-term rehabilitation service after illness or injury, either in a 24-hour rehabilitation centre or a patient's own home.

Health and social care professionals work together to support you in situations where you are suddenly unable to safely manage everyday activities due to illness or injury.

We provide a wide range of services such as physiotherapy, occupational therapy, district nursing, community nursing and assistance with personal care as needed. All professionals involved in your care attend weekly team meetings to keep up-to-date on your progress. Care co-ordination will oversee your care.

The service is available to people aged 18 and above who have a condition that is stable and can be managed safely in the community.
