Care Home Information
Our Speech and Language Therapists take referrals for care home residents with communication and/or swallowing problems who are registered with a Hull or East Riding GP.
We see adults with acquired communication and swallowing problems (dysphagia) caused by conditions such as stroke, cancer, Parkinson’s Disease, multiple sclerosis, COPD and frailty.
Swallowing Referrals:
We take referrals for swallowing from care home managers, deputy managers, team leaders and registered nurses. We don’t take swallowing referrals from carers or senior carers. We will send these back if they haven’t been countersigned.
Communication Referrals:
Referrals for communication can come from any care home professional, the person themselves or their relative.
Home Visits and Video Consultations
The majority of direct assessments are done as home visits. In some cases we may offer a video consultation. This is only done when the patient will be seen quicker by video than they would be face to face and where it is agreed with yourselves and the patient that it is appropriate. We will send specific information out about that assessment. Video consultations are as important as home visits. Missing a video consultation will delay not only your patient’s care but the care of others as we will have lost an appointment slot. If a resident has been admitted to hospital when an appointment is due, please let us know as soon as possible.
Please note that people with learning disabilities who need help with swallowing or communication are usually seen by the speech and language therapists in the Adult Learning Disabilities service provided by Humber Teaching NHS Foundation Trust.
