Patient Passport

This passport provides important information about a person’s needs, wishes, and include how they should be cared for when attending the hospital for an appointment or hospital stay.

The passport is not designed to hold all the information about a person.

It should have enough information to make sure staff are able to identify a person's reasonable adjustments and communication needs.

This passport should also be used to support patients at GP appointments and any other health appointments.
