Patient Passport
This passport provides important information about the reasonable adjustments a person’s needs when accessing health appointments or hospital stays.
The passport is not designed to hold all the information about a person.
It should have enough information to make sure staff are able to identify a person's reasonable adjustments and communication needs.
This passport should also be used to support patients at GP appointments and any other health appointments.
