Learning Disability Health Check Pre- Screen Tool and Health Action Plan

Patients can be sent this questionnaire with their annual health check invite letter or once they have booked their appointment. Patients can then complete this document and bring it with them.

This document will provide practice staff with a snapshot of any communication needs and reasonable adjustments that the patient may have. This information can then be added to the patient’s record for future appointments.  The pre -screen tool also includes a health action plan. This should be completed during the appointment by the practice staff and shared with the patient.
