Telehealth: changing healthcare, changing lives
05 February, 2018
Improving the health and wellbeing of people in our community is crucial here at CHCP CIC. Our Telehealth service offers just this through simple user-friendly device(s) that are designed to monitor an individual’s health on a daily basis, identifying problems before they become critical. Long praised for their ability to deliver healthcare services to those that otherwise couldn’t access them, telehealth devices are able to take readings that include oxygen levels, weight, pulse, temperature and blood pressure, with results sent direct to nursing staff and without leaving the house.
Whether it’s helping you to become more independent, recognise your symptoms by enhancing your knowledge, helping you to manage your condition, or avoiding unnecessary hospital admission or GP visits, telehealth is capable of increasing access to quality healthcare, anywhere on the globe.
Our service and daily monitoring on this 3-6-month educational programme, help to support people from living with Chronic Obstructive Pulmonary Disease (COPD) and Heart Failure, at the user’s convenience. Whether at home with family, or on holiday, our service has you covered and ICT-driven techniques like this are enabling us to improve on quality and efficiency.
Readings will allow our dedicated nursing team to assess people’s conditions through results sent direct from the machine, to the nurses. If there are any problems with readings, a member of the Telehealth team would contact the individual by telephone and advise further. Frequent, personalised health tips and medication reminders are even sent direct to users based on readings, allowing individuals to feel more in control of their own health and wellbeing.
