Anticoagulation Service
The nurse-led Anticoagulation Service provides an initiation, monitoring and dosing service to people who have been prescribed Warfarin. The nurses have specialist knowledge and experience about montoring your blood test (INR) and can advise you on the appropriate dose of Warfarin to take and when you need to return to have repeat blood tests.
You must be referred to the Anticoagulation Service by a health care professional (such as a doctor or nurse) and must:
- be 18 years or over
- registered with a Hull GP
- have an illness or condition that can be treated with anticoagulation therapy.
