Health and Social Care Team

What is the Service?

This service is made up of Health Care Assistants, who are skilled in the delivery of high quality specialist palliative nursing care and support within a patient's own home. The team help with personal care and comfort measures, provide psychological and emotional support to both patients and their carers, prepare light meals and help to provide respite breaks for carers.

Aim of Service

The service aims to ensure the provision of high quality care within a patient's own home and to treat every patient with dignity and respect.

Service Referral

Referrals are accepted from health and social care professionals and can be made through the CHCP CIC single point of access for specialist palliative care services telephone line on 01482 247111.

Referrals will only be accepted by patients who meet the following criteria:

  • Patients must be over 18 years old
  • Registered with a Hull GP
  • Have a medically identified palliative care need.

Expected Outcomes for the Service

To ensure that patients with a life limiting illness receive the highest possible level of individualised care within their own home, and allow patients to be cared for within their preferred place of care.
