Macmillan Community Therapy Service
The Macmillan Community Therapy Service provides a range of theraputic approaches which are designed to maximise independence, productivity and wellbeing, which in turn enables patients, carers and their families to live actively until death.
The service is made up of Physiotherapists and Occupational Therapists, and provide interventions such as fatigue management, relaxation and mindfullness activities, provide equipment, offer fast-track referral for major adaptations such as stairlifts, create individual exercise plans to maxmise and retain mobility and offer advice regarding breathlessness and breathing techniques. As a further activity, the service has recent acquired an allotment space and are offering it as a group activity to patients and their carers/families.
The service includes all palliative and end of life patients, regardless of their diagnosis.
Referral Criteria
- Patients aged 18 or over
- Registered with a Hull GP
- Patients who have a palliative diagnosis, requiring specialist theraputic intervention.
Referrals can be made by patients and all other healthcare professionals in all areas of care.
This can be done via a letter, fax or telephone.
