Specialist Palliative Care Services

What do we do?

We want patients with advancing illness to get the best possible palliative and end of life care, no matter what their diagnosis is.

Most people who are nearing the end of their life would prefer to be cared for in their home environment – getting the right care in the right place and at the right time.

The Specialist Palliative Care Services provides additional care resources and complements the care delivered to patients in their homes.

These services include:

  • Specialist Palliative Care Team – including Palliative Medicine Consultant, GP with extended role, Macmillan clinical nurse specialists, Macmillan pharmacists, Macmillan physiotherapy and Macmillan occupational therapy
  • Community specialist palliative care clinics
  • Palliative care co-ordinators
  • Health and social care team – healthcare assistants who provide additional supportive and hands-on care to patients at end of life and their carers
  • Palliative Night Care Service – health care assistants who stay with the patient to give them care and security between 10pm and 7am

How to refer and criteria

Referrals can be made by ringing our Care Co-ordination Centre on 01482 247111 24 hours a day, seven days a week.

Referral criteria:

  • Adults over the age of 18
  • Patients who have a medically identified palliative care need.
