Holy Name

Holy Name is relaxed and friendly, homely and comfortable. It’s not a hospital.

Holy Name provides short-term rehabilitation after an illness or injury. A wide range of health professionals
such as physiotherapists, nurses, care assistants and occupational therapists will help you to regain as much
independence as possible.

You will have regular assessments to decide how long you need the service for. This might be for a few days or until
your rehabilitation is complete. The assessments will be done by the people looking after you and others involved in your care and you will have regular reviews of your progress to check you’re happy to continue your rehabilitation at Holy Name.

If you need long-term support, we may refer you to the long-term team. This service is means tested, so you may
have to contribute to the cost.

What happens during my stay?

We will help you to become as independent as possible during your stay at Holy Name. You will take part in
therapy sessions and following the therapy team’s advice with support from our health care assistants.

You will need comfortable everyday clothing and some well-fitting footwear. You will also need to bring your
own toiletries.

We recommend that you only have a small amount of money with you and that you leave valuables at home.

We do have some facilities for safekeeping in exceptional circumstances but we do not take any responsibility for
any loss or damage of valuables/money/belongings.

Any mobility aids (such as walking sticks, wheelchairs and Zimmer frames) must be agreed with an Occupational
Therapist or a Physiotherapist and they must be brought in and labelled.

Any continence equipment or aids to help you with daily living must also be brought in during your stay. We do not provide continence products.

Discharge Planning

This is a term used to describe the preparations for you to leave Intermediate Care or the Reablement Service.
We start thinking about this as soon as possible so that you can leave safely when you are ready. As your health
and abilities improve you may need to move to one of our other rehabilitation facilities.

Please note that we will not discharge you without a full discussion between Holy Name staff, your family/carers
and yourself, or without the help and support that you may require. We are not able to offer temporary accommodation while re-housing issues are resolved.
