Community Children's Nursing
We are a team of Children's Nurses, Adult and Learning Disability Nurses, Health and Development Practitioners and Health Care Support Workers in a family friendly, needs-led service for children, young people, their families and carers.
We provide nursing assessment, treatment and support for children and young people aged 0-18 in the community across the Hull and East Riding area.
Our service is available in clinics across the region, at home, school or other appropriate venues where needed. We can also offer telephone advice, support and review.
Referral Process and Enquries
Referrals can be made by health professionals, social care and the voluntary sector.
Referrals and enquries are processed through the Single Point of Access Team for Community Nursing and Medical Services.
