Complex Nursing Care

We offer nursing support and advice for children and young people who have long term conditions, life limiting conditions and/or complex health needs.

End of Life Care: 24 hour on call nursing advice and support for children and young people who are nearing the end of their life.

Training Parents and Carers: Structured training and support for parents and carers utilising on-line resources, face to face practical demonstration and support, and competency packages.

Transition Planning: Support for young people aged 14 years and over with long term health needs, to help them move into adult health services.
