International recognition for Hull End of Life Care Team

City Health Care Partnership’s Specialist Palliative Care Clinic, in association with Dove House Hospice is celebrating after coming third in the multi-disciplinary teamwork category at an International Nursing Awards ceremony in London. The award from the International Journal of Palliative Nursing highlights inspirational work making a difference to the care of patients and their families and friends,

Palliative Care is aspecialised area of healthcare that focuses on helping people with a life limiting illness feel comfortable. This is an all round approach that could include things such as appropriate pain relief, making sure patients are in an environment they’re comfortable with, providing support for the patient as well as their friends and families,and generally enhancing quality of life.

To support patients and their families the innovative service, based at the Bilton Grange Health Centre on Diadem Grove, has been developed to provide assessment and treatment for all diagnoses closer to the patients’ home. The team includes a consultant, pharmacists, community matrons from a variety of service areas, Macmillan nurses and admin support, working together to manage complex symptom control, reducing the need for hospital admissions and allowing more patients to die at home, if this is their preferred place of care.

The clinic was launched in July 2010 as a two year pilot programme funded by NHS Hull. Referrals can be made by all health and social care professionals, voluntary agencies, patients and their carers.
