Evolve Referrals

If you are 18 or over and are registered with a Hull GP,  there are a number of ways you can be referred to Evolve:

You can refer yourself by phoning us on 01482 344083, completing the contact form below or by texting EVOLVE to 61825. You will be contacted by the service as soon as possible using your preferred method.

If you are currently being treated within the mental health services you can discuss with your care co-ordinator and they can support you to refer yourself or they can make a referral for you.

If you are under 18 please discuss your needs with your GP or the Child and Adolescent Mental Health Service (CAMHS).
